Students who still need one or more immunizations required for enrollment in public schools in Virginia need to provide proof of the immunization(s). The deadline for your student to meet this immunization requirement is August 15, 2025. If you fail to comply with providing the completed proof of this required immunization(s) by August 15, 2025, your student will not be permitted to start school on Monday, August 18, 2025.

Please read below and follow these two steps:

  1. Schedule an appointment with your health care provider or with the Prince William Health District for your child to receive the missing immunizations.
  2. Provide documentation to your student’s school indicating that your student has received the immunization(s). Please write your student’s full name and student ID number on the documentation. You can email the immunization(s) information directly to our nurse Kathy O'Shea at [email protected] or deliver a paper copy of the immunization(s) information to Patriot.

If your health care provider or the health district indicates your student cannot receive the immunization(s), please provide the school nurse with a signed statement for medical exemption from the health care provider. Religious exemption forms may also be submitted, and the form is available on the Virginia Department of Education website.

Thank you for your prompt attention. Please note it will take staff time to receive the delivery of paper or emailed documents and enter them into our student management system. Clinic staff are out of the building from June 12 until August 4.
